ACO Learning Collaborative:
Addressing Social Drivers of Health
The Institute for Accountable Care and NAACOS are supporting fourteen ACOs through a learning collaborative focusing on developing and implementing strategies to address Social Drivers of Health (SDOH). The collaborative is organized around seven specific content areas that are described below. Presentations and supporting materials for each session will be made available below.
